There is a space for comments, e.g if you are selling for someone else, and/or the chametz is overseas (or a combination of both, etc). Per the Rav's instructions, please clarify to the best of your abilities how access can be achieved to the chametz if necessary.
The Rav requests that all this be submitted by 8:00 PM on the night of bedikat chametz (7 April, 2020).
